Case in brief
A 19-year-old man with an aggressive osteoblastoma of the calcaneus underwent subtotal calcanectomy. Limb salvage was reconstructed with a custom 3D-printed titanium endoprosthesis designed to support subtalar and calcaneocuboid fusion and address ligamentous deficiency; the calcaneal tuberosity was reattached while preserving the Achilles tendon. The implant was incorporated by 3 months, permitting full weight bearing. At 18 months, there was no loosening or tumor recurrence, and the patient had excellent function and returned to his pretreatment activities.
What made this case unusual
The custom porous titanium implant was designed for calcaneal reconstruction after tumor resection, including fusion of both adjacent joints and reattachment of the calcaneal tuberosity with the Achilles tendon spared.
Diagnostic / clinical pearl
After subtotal calcanectomy for an aggressive calcaneal tumor, a patient-specific endoprosthesis can be designed to address both joint fusion and ligamentous deficiency while preserving the Achilles tendon.
