Case in brief
A 66-year-old woman presented with a left frontal lobe mass and underwent surgical resection. Histology showed a high-grade glioma with pleomorphism, brisk mitotic activity, vascular proliferation and necrosis, alongside oligodendroglial-like features, nuclear clusters and small rosette-like structures. Molecular testing identified several genomic alterations, including a SYN2::PPARG fusion; DNA methylation profiling classified the tumor as diffuse pediatric-type high-grade glioma, RTK1 subtype, subclass C. After surgery, she developed herpes simplex virus 2 meningoencephalitis and died.
What made this case unusual
The tumor was classified as a pediatric-type high-grade glioma in a 66-year-old adult, and testing identified a SYN2::PPARG fusion not previously reported in glioma.
Diagnostic / clinical pearl
In an older adult with an atypical high-grade glioma, integrating histology with molecular testing and DNA methylation profiling can clarify tumor classification.
