Treatment-resistant depression poses substantial clinical challenges, particularly following multiple unsuccessful pharmacologic treatments. This brief report considers the role of hope as a therapeutic imperative in the management of treatment-resistant depression.

Existing staging systems can help characterize the extent of treatment resistance and support a systematic approach to management but do not fully capture these humanistic dimensions. Hope extends beyond simple optimism to encompass a realistic belief in the possibility of meaningful improvement despite previous treatment failures.
